- One-year Fellowship in Pediatric Ophthalmology and Strabismus (For year 2020)
1. Prepare the following documents
· 1 pc 2x2 ID Picture
· Medical School General Weighted Average (GWA) True Copy
· Endorsement Letter from Department Chair or Training Officer (for Fellowship application)
· Philippine Board of Ophthalmology Certificate (for Fellowship application)
· Photocopy of the following: Transcript of Records, Medicine Diploma, Certificate of Internship, PRC Board Rating (may follow for board takers), Certificate of Residencey (for Fellowship application)
2. Obtain an application form. Get an application form from the Deputy Director for Health Operations Office of the Philippine General Hospital
3. Submit your forms. Submit your application form to the Deputy Director for Health Operations Office of the Philippine General Hospital. For Fellowship applicants, submit your Endorsement Letter to the Chairman’s Office, 5th Floor, Sentro Oftalmologico Jose Rizal
